No explorer in Vista = Trojan-Backdoor-Prorat-D
Solved !!
Had the same problem, turned out to be infected with Trojan-Backdoor-Prorat-D.
Nasty bugger, but was able to get it out with Webroot spysweeper.
Heres the removal instructions
